Monitoring of immune function in critically ill children with influenza reveals severe immune suppression in non-survivors

Jan. 21, 2013 ? Investigators from 15 children's medical centers, including Nationwide Children's Hospital, observed and evaluated critically ill children with influenza to evaluate the relationships between levels of systemic inflammation, immune function and likelihood to die from the illness. The study appears in the January issue of Critical Care Medicine.

The innate immune system is the cellular arm of the immune system that serves as a first-responder to new threats, and is thought to drive the inflammatory response in many forms of critical illness. Recent evidence indicates that suppression of innate immune system function can occur in critically ill patients.

This immune suppression can be quantified in the laboratory through measurement of the capacity of the patient's blood to produce a specific cytokine, tumor necrosis factor (TNF)-? , when stimulated with lipopolysaccharide (LPS), a substance which should induce a robust TNF ? response. Patients with innate immune suppression produce reduced amounts of TNF? when their blood is stimulated. In its most severe form, this condition is known as immunoparalysis. Severe reductions in TNF? production capacity have been associated with the development of secondary bacterial infections and death in critically ill adults and children.

Immune function is not routinely measured in patients with influenza, though some therapies used in this population, such as corticosteroids, can be potently immunosuppressive.

"Both pro- and anti-inflammatory therapies have been proposed as additional treatment options for influenza infection," explains lead study author Mark Hall, MD, Critical Care specialist and principal investigator in the Center for Clinical and Translational Research of The Research Institute at Nationwide Children's Hospital. "However, a lack of immune monitoring data in the pediatric population has made therapeutic decision-making difficult in children."

In this first-of-its-kind, multicenter observational study, blood samples from critically ill children with influenza were tested using highly standardized techniques to determine the capacity of the participants' innate immune system to produce TNF? when stimulated with LPS. Healthy control subjects also had their blood tested for the same properties.

Results indicated that despite high levels of circulating pro-inflammatory cytokines, critically ill children with influenza demonstrated lower TNF? production capacity compared with healthy control subjects. Further, children who died from influenza had markedly lower TNF? production capacity compared with survivors. Patients who were co-infected with influenza and the bacteria Staphylococcus aureus showed the greatest degree of immune suppression.

While the reduced capacity to produce TNF? among the critically ill children compared to healthy subjects was expected, the degree of reduction in capacity was severe enough to be highly predictive of death from the illness.

"The study demonstrates a strong relationship between mortality and reduced innate immune responsiveness in critically ill patients," said Dr. Hall, also a faculty member at The Ohio State University College of Medicine. "It also demonstrates the feasibility of large-scale immune monitoring of the kind necessary to develop and test therapies for these critically ill children. The identification of potential treatment thresholds is important because strong evidence suggests that innate immune suppression associated with critical illness may be reversible."

Advocating for additional studies, investigators suggest that patient-specific immune monitoring could aid in determining the most effective treatment for these critically ill patients. Therapies that stimulate the immune system may have a significant role in the treatment of high-risk children with severe immune suppression associated with influenza infection.

Ukrainian ex-PM suspected in murder case

KIEV, Ukraine (AP) ? Ukrainian authorities have formally notified jailed former Prime Minister Yulia Tymoshenko that she is a suspect in the murder of a businessman and lawmaker in 1996.

Tymoshenko is suspected of "ordering and organizing" the contract-style murder of Yvhen Scherban, his wife and two other people, the Prosecutor General's Office said on its website Friday. Prosecutors say the killing appeared aimed at eliminating a business rival.

The move stops short of formally charging Tymoshenko, but indicates charges are likely. Prosecutors have said for months that she was being investigated in the case. Tymoshenko denies the accusations and says it is part of a broader campaign by her foe, President Viktor Yanukovych, to keep her in prison and bar her from politics.

Tymoshenko is serving a seven-year prison term for abuse of office while negotiating a gas deal with Russia in 2009.

The new case was likely to further spoil Ukraine's ties with Western nations, which have called Tymoshenko's imprisonment politically motivated. The European Union has frozen a key cooperation deal with Ukraine over Tymoshenko's imprisonment.

Tymoshenko's party said a formal announcement of suspicion represented a "shameful day in the history of the Ukrainian law enforcement system."

"Their fear has paralyzed their mind. They are no longer hiding the fact that they want to lock away for life not only the leader of the opposition, but also the whole of Ukraine," the party said in a statement.

The European Court of Human Rights is expected to rule on the Tymoshenko case in the next several weeks, and analysts said the murder case was the government's attempt to keep her in jail during the 2015 presidential campaign if the European court rules in her favor in the gas case.

"It is evident that the Ukrainian authorities are really and seriously intent on keeping Tymoshenko in jail," said Vadim Karasyov, a political analyst with ties to the government, according to the Interfax news agency.

Meanwhile, Tymoshenko's family and allies have expressed concern over the state of her health. Tymoshenko is undergoing hospital treatment for a severe back condition. Her lawyer, Serhiy Vlasenko, said Friday that when he tried to see her recently, she was unresponsive for several minutes. Doctors insisted she was in a satisfactory condition and was under the influence of sedatives.

Tymoshenko's office said Saturday that they have been unable to reach Tymoshenko and inquire about her health since that incident.

SYDNEY - Indicted Internet entrepreneur Kim Dotcom formally unveiled his new file sharing and cloud storage service Mega in a press conference from his mansion near Auckland, New Zealand on Sunday, arguing that it was legal and made possible because of the shutdown of Megaupload.

"This fresh start for Mega is a good thing,? the German-born, New Zealand resident said during the press conference, which was live streamed online ahead of what was billed as a lavish Sunday night launch party for the new site at his mansion.

He even argued that the downfall of his former website Megaupload made the new site possible. "It wouldn't have even been possible to do this," news organization Fairfax NZ quoted him as saying. "No one else who is currently in business in the file storage arena can just update their site and be like us, you have to start from scratch."

In an interview with Reuters, meanwhile, Dotcom emphasized that the new site launch was not meant to be revenge on Hollywood or U.S. authorities. "This is not some kind of finger to the U.S. government or to Hollywood," he said.

And he added: "Legally, there's just nothing there that could be used to shut us down. This site is just as legitimate and has the right to exist as Dropbox, Boxnet and other competitors," he said in reference to well-known cloud storage services.

At one point during his Sunday press conference, Dotcom even said his goal was to list the new business, which uses the company name The Privacy Company, on the New Zealand Stock Exchange.

Ahead of the launch of Mega, the Motion Picture Association of America, which claims copyright infringement by Megaupload, remained skeptical.

"We are still reviewing how this new project will operate, but we do know that Kim Dotcom has built his career and his fortune on stealing creative works," the MPAA said in a statement cited by Fairfax NZ. "We'll reserve final judgment until we have a chance to take a closer look, but given Kim Dotcom's history of damaging the consumer experience by pushing stolen, illegitimate content into the marketplace, count us as skeptical."

The Mega site launched Sunday morning New Zealand time, and within several hours over 250,000 users had registered for the site, according to Dotcom. As the site went live, he tweeted: "As of this minute one year ago #Megaupload was destroyed by the US Government. Welcome to Mega.co.nz?.

Dotcom and several associates were arrested during a raid on his mansion in outer Auckland a year ago on charges of racketeering, copyright infringement and money laundering. The charges said that Doctom?s then-website, Megaupload, enabled Internet piracy and illegal downloading through its file sharing service.

Hearings about a possible extradition to the U.S. are due to be heard in New Zealand in March. Dotcom said Sunday he was hoping to stay in New Zealand if the extradition proceedings fail.

"My biggest fear is if we succeed, will there be an end - will that be enough to say, 'OK, we made a mistake, leave this guy alone' or will the people in government and the senior people in law enforcement take this defeat and be bitter about it and try and find something else,? he told the press conference.

The new Mega site has been designed by Dotcom and Mathias Ortmann, Bram van der Kolk, and Finn Batato, Dotcom?s co-accused in the Megaupload case, while industry veteran Tony Lentinos serves as the CEO.

Mega.co.nz, like Megaupload, allows users to store and share large files, providing registered users with 50 gigabytes of free storage. The key difference is an encryption and decryption feature for data transfers. The decryption keys for uploaded files are held by the users, not Mega, which means the company can't see what is in the files being shared. Dotcom can therefore argue that Mega can't be held liable for content it can not see, according to Fairfax NZ.

Sharpening knives is one of those tasks that superficially seems easy, but involves a bit of geometry to get it right. Different blade edges have different types of grinds which require slightly different angles to sharpen properly, otherwise you can take off too much metal and possibly ruin the grind, requiring even more metal removal to fix.
